Madison Creasman

Social Care Program Coordinator, Impact Primary Care Network

Madison is a passionate healthcare professional who has made a significant impact on the communities she serves in Western North Carolina (WNC). As the social care program coordinator for Impact Primary Care Network (IPCN), Madison leads the development of care management programs focused on social determinants of health (SDOH) and the Healthy Opportunities Pilot (HOP) for the network’s primary care practices. She also creates training programs for IPCN and primary care staff to support SDOH and HOP screening, referrals, and follow-up.

Having lived in WNC since she was two years old, Madison has deep ties to the region. In her recent role as a community outreach and HOP specialist with Mission Health Partners, she developed a strong understanding of the area’s needs and gained valuable insights on the growing HOP program. In fact, Madison was the first person in North Carolina to submit a HOP referral.

Madison holds a bachelor’s degree in public health with a community health concentration from East Carolina University (ECU) and is currently pursuing a master’s in public health at ECU with a focus on policy, administration, and leadership.

In her free time, Madison enjoys exploring new eateries with friends or relaxing at home with her cat, watching TV, or reading.
