Renee Cooper

Care Navigator

Renee Cooper brings over 20 years of experience in human services to her role as care navigator at IPCN. She supports individuals by providing care coordination, conducting outreach and follow-ups, and assisting with access to health-related social needs.

She previously served as a community health worker (CHW) lead, supervising a team of CHWs while managing a caseload, with a focus on care transitions, hospital and ER follow-ups, and closing care gaps, at Hopscotch Primary Care. Her work included addressing social drivers of health, connecting individuals to community resources, conducting home visits, and supporting high-risk populations. As both a certified community health worker and a peer support specialist, she is also trained in recovery support and trauma-informed care.

Passionate about meeting people where they are, Renee is dedicated to building trust, advocating for patient needs, and helping individuals navigate both healthcare and community systems. Outside of work, she is deeply involved in recovery outreach and community service, creating spaces where people feel seen, supported, and empowered.
