With IPCN’s support, they gain access to advanced care management, seamless coordination, quality improvement strategies, and population health initiatives — all designed to improve health outcomes and drive down costs. These services include:
- Access to IPCN’s care management platform with implementation and training support
- Assistance with PHP and EHR data exchange and reporting compliance (if applicable)
- Support for quality gap reporting and data analytics
IPCN Care Management
IPCN will deliver care management, care coordination, quality improvement efforts, and population-based initiatives aimed at enhancing health outcomes and reducing healthcare costs for beneficiaries. This includes:
- Access to IPCN’s care management platform, including implementation and training support
- Assistance with compiling and providing data to comply with CMS care delivery data exchange and reporting requirements
- Availability of e-consult capability (pass-through cost)
