Population Health

Impact Primary Care Network (IPCN) supports its participating practices and providers by helping them to operate more efficiently and effectively by leveraging health data and analytics. This enables our practices to close gaps in care, achieve quality measures, and comply with reporting requirements. We help practices shift to value-based payment models and help their patients optimize their health through improved access to quality care management.

IPCN offers flexible participation options that empower practices to join our network in a way that best suits your business needs. Click here for participation options.

Medicaid Managed Care

IPCN provides quality care management services for practices participating in NC Medicaid Managed Care and other initiatives. Our data analytics platform and quality program consulting can help you deliver Medicaid Managed Care efficiently.

We help primary care practices improve their patients’ and community’s health outcomes by:

  • Providing a robust care management platform
  • Coordinating specialty care and empowering patients through self-management
  • Leveraging a comprehensive network of human service providers

Value-Based Care

The transition to value-based care and payments—emphasizing prevention and improved population health outcomes over traditional service delivery—has paved the way for broader efforts to address upstream drivers of health.

Increasingly, healthcare policymakers recognize the significant impact of health-related social needs, and how addressing these factors can improve population health outcomes.

“Being part of IPCN has allowed me to truly focus on whole-person care. The network’s support in integrating clinical and social services has made a real difference—not just for my patients’ health outcomes, but also for how supported I feel as a provider. It’s empowering to be part of a physician-led effort that’s designed with our local community in mind”.
