Impact Primary Care Network (IPCN) is a physician-led, clinically integrated network working to transform healthcare in Western North Carolina (WNC). Our nonprofit network supports whole-person health through quality care management that helps participating practices integrate clinical and social care. IPCN prioritizes the needs of WNC providers and their patients and is deeply invested in the lasting health of our rural region.
IPCN was established by Impact Health in 2024 in response to the needs of providers and the community.

IPCN is developing effective solutions to primary care challenges, aiding practices in delivering high-quality accessible care through Medicaid and Medicare initiatives. Our care management services help providers improve the patient experience and health outcomes for the people of WNC.
Working with Impact Health, IPCN provides care management for participating practices. IPCN also offers a user-friendly, best-in-class, care management platform to help practices access powerful data analytics to enhance population health and reduce unnecessary costs.
IPCN has dedicated care managers who work hand-in-hand with healthcare providers to help achieve their patients’ health goals. Care management services are provided at no cost to patients through IPCN practices and include everything from support with managing medication to accessing critical social needs like healthy food, safe housing and healthy relationships.
Our network also offers sub-delegated care management services to support practices that prefer to use their own in-house care management teams.
